Background: In hypertrophic cardiomyopathy (HCM), one of the main pathophysiological features is diastolic dysfunction. Diastolic dysfunction detected by restrictive filling pattern was associated with worse outcomes in patients with HCM. However, atrial fibrillation (AF), which is the most common supraventricular sustained arrhythmia in HCM, makes it difficult to accurately evaluate diastolic function. The prominent mid-diastolic filling wave (mitral L-wave) has been indicated advanced diastolic dysfunction even in patients with AF. However, the association between mitral L-wave and outcome in HCM patients remains unclear. We thus evaluated the prognostic implications of mitral L-wave in HCM patients. Methods: This study included 445 HCM patients. The mitral L-wave was defined as a distinct mid-diastolic flow velocity following the E wave with a peak velocity >20 cm/s. The endpoint of this study was the combination of sudden cardiac death, successful resuscitation after cardiac arrest, appropriate implantable cardioverter-defibrillator shocks, heart failure death, and stroke-related death.
