Background:Reduced ejection fraction (EF) is one of the risk for sudden cardiac death (SCD). We can use the wearable cardioverter-defibrillator (WCD) as a bridge therapy to prevent SCD. When we decide to implant cardioverterdefibrillator, we usually re-evaluate patients´ condition after appropriate treatment.Methods and Results: We used the WCD for 48 patients from April 2015 to September 2018. The mean age was 55 years old and mean initial EF was 33±11%. All patients had received appropriate therapy such as medication, re-vascularization, catheter ablation and surgery, and they were instructed to use WCD at discharge. 38 patients used the WCD as primary prevention for SCD. Only 1 patient (2%) received appropriate shock, and all patients did not receive inappropriate shocks from WCD. Moreover, their LVEF improved significantly after optimal treatment in most patients with LVEF of 35% or less on admission (Figure). Four patients (8%) were implanted cardioverter-defibrillator after removing WCD. Conclusions: WCD is effective as a bridge therapy to prevent SCD.
