概要 Systemic sclerosis (SSc) is a complex disease with multi‒organ tissue fibrosis including the myocardium. Myocardial involvement is associated with poor prognosis in patients with SSc. Taken that the detection of left ventricular systolic abnormalities by speckle tracking echocardiography has not been well established, we aimed to evaluate myocardial strain profile in SSc patients.
Methods and Results: Thirty‒five SSc patients (mean age 60 years) without significant valvular heart disease and 35 healthy controls were studied. A comprehensive echocardiographic examination was performed in all participants. Left ventricular end‒diastolic volume (EDV), end‒systolic volume (ESV) and ejection fraction (EF) were assessed by biplane Simpson's method. We measured the global longitudinal strain (GLS) from the apical long‒axis, and two‒ and four‒chamber views. There were no significant differences in EDV, ESV and EF in both groups. Conversely, GLS was significantly lowerin SSc patients when compared with controls (‒15.3% vs.‒16.9%, p=0.001).
Conclusions:SSc patients showed subclinical left ventricular dysfunction. GLS might be useful method for detecting. myocardial dysfunction prior to the development of cardiovascular symptoms.
