発表タイトル Fontan Palliation with a Biventricular Heart Has Potential Ventricular Deterioration Depending on Dominant Ventricular Characters

概要 Introduction: Fontan palliation is occasionally performed for the congenital anomalous heart with two ventricles that cannot be surgically separated. It remains unclear whether or not the wall motion of biventricular Fontan is similarly to that of single ventricular Fontan circulation. We assessed the wall motion and potential deterioration depending on dominant ventricular characters.
Methods: Prospectively, we enrolled consecutive patients with biventricular Fontan palliation and performed CMR. We divided there patients into 2 groups: LV or RV dominant biventricular Fontan and compared global longitudinal strain (GLS), global circumferential strain (GCS), early diastolic strain rate (SR) and ventricular torsion using feature tracking.
Results: 40 biventricular Fontan patients (20.6+/‒8.5 yrs) [26 patients with dominant RV and 14 patient with dominant LV]. GCS and ventricular torsion in LV dominant biventricular Fontan were better than those in RV dominant biventricular Fontan (15.7+/‒3.8% vs 13.0+/‒2.9%, 4.4+/‒2.9 º/cm vs 2.3+/‒0.86 º/cm). LV dominant biventricular Fontan showed the tendency of better GLS compared to RV dominant biventricular Fontan (15.7+/‒3.3 vs 13.2+/‒3.4, P=0.07). There was no significant difference in age, BNP, QRS duration, early diastolic peak SR, EDVi, EF or CI.
Conclusions: LV dominant biventricular Fontan shows better circumferential wall motion and torsion compared to RV dominant biventricular Fontan. Biventricular Fontan palliation has potential ventricular deterioration depending on dominant ventricular characters.
