概要 Background: The gender difference in long-term clinical
prognosis after percutaneous coronary intervention (PCI)
with rotational atherectomy (RA) against severe calcified
coronary artery is totally unclear in the drug-eluting stent
(DES) era.Methods: This is the subanalysis of J2T-ROTA
registry that included 1,090 patients undergoing PCI with
RA at three hospitals between 2004 and 2014. After the
exclusion of patients receiving regular hemodialysis, 788 patients including 570 males and 218 females were finally analyzed. The primary endpoint was major adverse cardiovascular event (MACE) including any death, acute coronary syndrome (ACS) and stroke. Results: Female patients
were significantly older, and frequently had chronic kidney
disease, presentation of ACS, atrial fibrillation (AF), lower
body mass index, poorer lipid profiles and higher levels of
C-reactive protein and brain natriuretic peptide than male.
Kaplan-Meier analysis showed a significantly higher MACE
rate in female (Log-rank: p=0.019) during 1,492 days of
median observational period. Multivariate Cox regression
analysis revealed that age, comorbidity of AF and values of
left ventricular ejection fraction were independent predictors
for MACE (HR: 1.07 [95%CI: 1.03-1.11]; HR: 1.91 [95%CI:
1.05-3.50]; HR: 0.95 [95%CI: 0.93-0.97], respectively). However, female gender was not independently associated with
MACE.
