Background: Patients with familial hypercholesterolemia (FH) have high risk for cardiovascular disease (CVD). But, it is not well known about age and sex difference of CVD risk between FH and dyslipidemia (DL) in Japanese. Methods: We retrospectively examined 438 patients suspected of FH from 2006 to 2018. The clinical heterozygous FH (HeFH), probable HeFH and DL diagnosed according to FH guidelines of Japanese Atherosclerosis Society were analyzed in this study. A total of 180 patients were performed genetic analysis and diagnosed according to gene mutations. We compared the adverse CV events (CV death, readmission with heart failure, coronary event) between HeFH and DL in each sex and age group (male < 65y [YM] or ≥65y [OM], female < 55y [YF] or ≥55 [OF]).Results: The total number of HeFH was 197 (56 in YM, 66 in OM, 28 in OF, and 47 in YF), and DL was 103 (56 in YM, 66 in OM, 28 in OF, and 47 in YF). There were no notable difference about coronary risk factors with each groups. In both OM and YM, HeFH were higher rate of the adverse events than DL (16.7% vs 7.7%, 12.5% vs 6.7%, respectively, p<0.05). In both YM and YF, the CV event also higher in HeFH than DL (10.7% vs 4.0%, 8.5% vs 0%, respectively, p<0.05).Conclusion: Regardless age and sex, HeFH have more than twice times higher risk of CVD than DL.
