Summary Background: Clinical observation suggests that arrhythmogenic right ventricular cardiomyopathy/dysplasia (ARVC/D) could have four different clinical phases: the concealed phase, electrical phase, failure phase and end-stage phase. However, these clinical phases are fully elucidated during long term follow-up period.Methods and Results: Between 1974 and 2012 at a tertiary center in Japan, we enrolled 90 patients in whom ARVC/D was diagnosed based on the modified Task Force Criteria 2010. During follow up period of 10.2±7.1 years, 47 patients experienced ventricular arrhythmia, 28 were hospitalized for heart failure and 21 patients died(Figure). Of the 28 HF hospitalization patients, 24 (86%) had experienced ventricular arrhythmia prior to HF hospitalization. Conclusion: Clinical phases vary with stage of disease, such as life-threatening ventricular arrhythmia, heart failure hospitalization, and death.
