Background: Chronic kidney disease (CKD) is highly prevalent in peripheral artery disease (PAD) patients, and majority of those suffer renal anemia. We ought to test whether the prognostic value of preoperative anemia depends on the severity of CKD in PAD patients after endovascular therapy (EVT). Methods & Results: This observational study included 475 consecutive patients with CKD grade≥3 who received EVT. They were divided into 4 groups depending on CKD degrees (grade 3 and 4/5) and comorbidity of anemia (hemoglobin level <11 g/dl); Mild-CKD/Non-anemia (n=136), Mild-CKD/Anemia (n=28), Severe-CKD/Nonanemia (n=195), and Severe-CKD/Anemia groups (n=116). During 824 days of median follow-up, there were 84 all-cause deaths (ACD). Kaplan-Meier analysis revealed that anemia was associated with ACD in Mild-CKD patients, but not in Severe-CKD patients (Figure). In multivariate analysis, Mild-CKD/Anemia group showed a significantly higher hazard ratio (HR) of ACD than Mild-CKD/Non-anemia group, even after adjustment for confounders (HR: 9.06, 95% confidence interval 1.67-49.2, p=0.011).Conclusion: Preoperative anemia increased ACD in PAD patients with mild-CKD but not in those with severe-CKD after EVT.
