概要 | Introduction: Although the survival rate of individuals with single-ventricular hearts has been dramatically improved by the Fontan operation, the long-term prognosis is as yet unclear.
Purpose: To clarify the long-term outcomes of the Fontan operation and to describe the clinical condition of long-term survivors. Design and Setting: Retrospective observational study set at a single institute. Methods: We analyzed 628 patients who had undergone the Fontan procedure in our institute between 1974 and 2013. Results: Overall, survival rate declines about 10% every 10 years. Forty-five patients underwent the Fontan operation more than 30 years ago; of these, 20 are alive and 25 have died. All long-term survivors had heart failure (NYHA I:0, II:17, III:3, IV:0). Nineteen (95%) of the 20 long-term survivors had a history of atrial tachycardia and 11 (40%) have had at least one thromboembolic event. Conclusion: Prognosis of the Fontan operation has improved in recent years. Further refinement of this treatment strategy is needed to improve event-free survival. |
