概要 | Introduction: Patients with congenital heart disease corrected by Fontan‒operation have liver congestion (LC) that progresses to fibrosis with late complications.
Objective: To investigate whether LC can be predicted by flow dynamics in Fontan-passway. Methods: 28 patients after arterio-pulmonary connection Fontan-operation who underwent cardiac MRI and the following contrast-enhanced CT were enrolled. We developed a vortex flow map that can quantify vortex flow index (VFI, %) in Fontan-passway from conventional cine MRI (Figure). LC was identified as abnormal parenchymal enhancement and cirrhotic signs on CT. Results: VFI was significantly smaller for patients with LC than those without (9% vs. 26%, p<0.01). The optimal cutoff of 16% VFI can predict LC patients, with 82% sensitivity and 80% specificity. Conclusion: LC after Fontan-operation can be predicted by vortex flow in Fontan-passway. |
