概要 | Background:Ventricular stiffness has been reported to be involved in heart failure owing to left ventricular diastolic dysfunction. However, the underlying mechanism and clinical significance of ventricular stiffness in patients with a single ventricle after the Fontan procedure are still unclear.Methods:We retrospectively studied 39 patients (mean age, 25.7 years; mean postoperative duration, 18.2 years) with a single ventricle after undergoing the Fontan procedure between July 2013 and September 2015. We examined the correlation between ventricular stiffness and the clinical profile of the patients. We calculated ventricular stiffness by using the following formula: (ventricular end-diastolic pressure [mm Hg] − ventricular minimum pressure [mm Hg])÷ventricular stroke volume (mL) × BGA (m2).Results:Ventricular stiffness showed a negative correlation with 6-minute walk distance (p = 0.008). Ventricular stiffness showed a positive correlation with plasma renin activity (p = 0.02) and central venous pressure (p = 0.046), and was significantly worse in the patients with the right ventricle (p = 0.02) and more moderate atrioventricular valve regurgitation (p = 0.009). In the multivariate analysis, ventricular stiffness was significantly worse in the patients with more moderate atrioventricular valve regurgitation (p = 0.03).Conclusions:In patients with a single ventricle after the Fontan procedure, ventricular stiffness was likely to be related to renin activation and atrioventricular valve regurgitation, which has a negative effect on exercise capacity. |
