概要 | Background. The objective of the present study was evaluation of the diagnostic efficacy of proton MRS for grading of intracranial gliomas.Methods. Original classification of the pathological MR spectra had been applied for evaluation of 38 low-grade and 33 high-grade intracranial gliomas. In each case long-echo single-voxel proton MRS was done.Results. Eight different types of the pathological MR spectra were noted in the present series, and 6 types had statistically significant association with tumor grade. Such metabolic patterns were identified in 45 cases (63%) and their identification had 0.78 and 0.83 cumulative positive predictive values for presence of low-grade and high-grade neoplasms, respectively. However, Type II A spectra with predominance of Cho and absence of Lac and Lip peaks (8 cases) and Type II B spectra with predominance of Cho, presenceof Lac peak, but absence of Lip peak (18 cases) did not permit reliable tumor grading. In such cases additional evaluation of NAA/Cho ratio with cutoff at 0.6 slightly improved diagnostic accuracy.Conclusion. In approximately 2/3 of cases pattern analysis of long-echo single-voxel proton MRS provides diagnostic information for reliable grading of intracranial gliomas. |
