Title Current Management of Central Venous Access at University Hospitals and Community Support Hospitals

Summary We conducted a current survey on central venous catheter insertion and management methods for a total of 149
university hospitals and regional medical support hospitals, and 47 facilities responded.
Regarding the insertion or removal reporting system for central venous catheters (CVC), 10 facilities has both mandatory insertion and removal reports, and 5 facilities has only insertion reports. 16 facilities answered that they have a qualification system for central venous catheter insertion. In response to the standard procedure of central venous catheter insertion procedure, especially the question of whether to use sonography, 6 facilities answered to use it at only pre-scan (sonographic scanning to confirm the target vessel position or condition before CVC insertion), and 21 facilities answered to use the real-time ultrasound guided method. In addition, the implementation status of the recommendations of the first report and recommendations for central venous catheterization from Japan Medical Safety Research Organization in 2017 was 50% or more for 36 facilities and less than 20% for 11 facilities. As the questions for the implementing status of the recommendations, 32 facilities cannot implement due to the shortage of the human resources, 25 facilities due to secure funds, and 29 facilities due to need to procure training equipment. Other than that, the contents of the recommendations are made known to doctors. The need for collaborators to implement the recommendations was mentioned.
