概要 Background: Recently myocardial crypts are detected by cardiovascular magnetic resonance(CMR). A higher prevalence of crypts in patients and gene carrier with hypertrophic cardiomyopathy(HCM) was shown and crypts seem relatively common in the normal population in western countries. Purpose: We investigate the prevalence and characteristics of myocardial crypts in Japanese using CMR. Methods: We examined retrospectively 310 consecutive patients referred for CMR. Crypts were defined as >50% invagination into normal myocardium. We performed MRI including cine and late gadolinium enhancement(LGE) imaging. We investigated the prevalence of crypts, location of crypts and LGE using a 17-segment model, and family history(FH) of patients with crypts. Results: Crypts were identified in 24 patients(7.7%). Thirteen patients were with HCM(54.2%), 7 with cardiomyopathy other than HCM, and 1 with congenital, valvular, coronary artery disease, and normal heart. The prevalence of crypts was significantly higher in patients with HCM(13 of 92:14.1%) than those without HCM(5.0%)(P=0.012). Six patients had multiple crypts. Most popular location was LV basal–inferior wall(19 patients). LGE was found in 10 patients and 2 had LGE in the same segment with crypt. Seven patients had FH of sudden death or LV hypertrophy or cardiomyopathy and 6 of them were patients with HCM. Conclusions: Myocardial crypts were more frequently seen in patients with HCM and also found in patients with other cardiac diseases.
