概要 Aim: To assess voltex flow (VF) patterns and the influences on RA thrombus and supraventricular tachycardia (SVT) in AP connection type Fontan using a novel MRI technique.
Method: Retrospectively, we visualized circumferentially and radially turbulent flow in the RA, and quantitated it into the range of ‒20% to 50%. Strong VF area (warm colour percentage from +35 to +50%) and weak VF area (‒20 to ‒10%) were measured on cine MRI. Results: 27 patients [25.1+/‒ 9.2 yrs] were enrolled. 15 patients had complications (SVT and/or thrombus) (Group A) and 12 patients did not have them (Group B). Circumferentially strong VF area in group A was significantly smaller than it in group B, whereas circumferentially weak/strong VF ratio in group A was significantly larger than it in group B. On uni‒ and multivariate logistic analyses, circumferentially weak/strong VF ratio on a transaxial image, and serum total bilirubin level were important factors of complication in AP connection type Fontan (odds ratio 1.12 and 46.7, 95%CI 1.007‒1.26 and 1.43‒1524.9, P value 0.04 and 0.04).
Conclusions: Circumferentially weak/strong vortex flow ratio on a transaxial image could be one of the surrogate makers of SVT and/or thrombus in adolescents and adults with AP connection type Fontan. This novel technique, VF mapping is useful to detect blood stagnation using conventional 2D cine MRI.
